MyPlate Assessment Rubric 
1. Students will use 3 paper plates to create their own MyPlate, according to the information in their text. 2. They will draw, print, or cut out pictures of foods that represent the correct portions and food groups on MyPlate. 3. Students will also label each food group with serving sizes to illustrate proper portions of each group. 4. Finally, they will write a reflection paragraph for each plate (one per meal) answering three questions.

Contains good creativity. Lots of colors, shapes, and appealing design. Shows the student spent time creating an artistic piece.
Exceeds

Eye catching. Great use of colors, shapes, and spacing on poster. Artwork is neat. Writing is easy to read.
Reflection paragraphs

-Neat work.
-Free of spelling and grammar errors.

Poor

Not completed and/or required plate or reflection questions were not answered at all. There are 6 or more spelling/grammar errors.
Fair

Reflection paragraph lacks answers to some of the questions posed. There are 3-5 spelling/grammar errors.
Good

Reflection paragraph adequately answers all questions posed. There are 1-3 spelling/grammar errors.
Exceeds

Reflection paragraph shows thoughtful insight and answers all questions posed. There are no spelling/grammar errors.
