Team Care Plan Creation Rubric 
Team Work will be evaluated on the quality of the submission and the degree that the individual participated in the teamwork. This project will the clinical documentation of the recognized responsive behavior(s), the reflection on possible triggers and ways to mitigate the triggers, pharmacological and non-pharmacological interventions, a succinct SBAR description, and recommendations for ongoing care to help reduce the client's responsive behaviors.
