Student returned from break with visual prompt and demonstrated:

willingness to complete work for up to 2 minutes.

Student returned from break following passive prompt (timer or visual cue) and demonstrated

willingness to complete work for up to 5 minutes.

Student did not initiate or complete the assignments even with prompting from the teacher.
Basic

Student initated the task, but became distracted and needed frequent redirection from the teacher.

Student initiated and completed the assignment, but needed one redirection from the teacher.

Student completed the assignments and was not a distraction to other students working around him/her.

Student did not participate in the activity at all upon returning from sensory break

Student was unable to follow directions due to being off task or unwillingness to work
Basic

Student followed the directions, but only minimally.
Mastery

Student completely followed all directions. When confused, student asked teacher for help in understanding the task.
Advanced

Student followed all directions when requested
