MyPlate Assessment Rubric 
1. Students will use a nine-inch paper plate to create their own MyPlate. 2. Color plate to resemble the colors on MyPlate. 3. Label each food group and cut or draw foods (at least two foods per group) that represents MyPlate food groups.

The work done meeting all expectations and shows that the learner is proud of his/her work. The effort that was put into this task is the best it can be by the learner.
Creativity

Lacks creativity. Not good use of space. Messy. The student demonstrated limited understanding of the foods in each group.
Fair

Not very appealing. Limited use of materials. Not a good understanding of the foods in each group.
Good

Contains good understanding of the food groups but did not include a variety of foods in each group.
Excellent

Eye catching. Good display of the 5 food groups. The student displayed a knowledge of the food groups by their choice of foods.
