Background Data Form	Mastered Demonstrates the ability to construct a clear and insightful problem statement with evidence of all relevant contextual factors. Adds in relevant notes regarding the student to clearly illustrate the case.	Working towards Mastery Demonstrates the ability to construct a problem statement with evidence of most relevant contextual factors, and problem statement is adequately detailed.	Needs Improvement Begins to demonstrate the ability to construct a problem statement with evidence of most relevant contextual factors, but problem statement is superficial.
Data Collection	Mastered Shows evidence of following all protocol directions; all data included on Administration Results Sheet. Multiple conditions given (ie, silent read, adult reader, and text reader). Obtains student rationale for accommodation preference.	Working towards Mastery Shows evidence of following protocol directions; most data included on Administration Results Sheet. At least 2 testing conditions were recorded. Student ratings present, but rationale not obtained.	Needs Improvement Shows little evidence of following protocol directions; few data included on Administration Results Sheet. Narrow use of testing conditions and/or student ratings/rationale.
Summary/Recommendations	Mastered Summarizes and makes recommendations in a manner that addresses thoroughly and deeply multiple contextual factors of the problem.	Working towards Mastery Summarizes and makes recommendations in a manner that addresses multiple contextual factors of the problem in a surface manner.	Needs Improvement Summarizes and makes recommendations in a manner that addresses the problem statement but ignores relevant contextual factors.
