Preparedness/Rehearsals	Excellent I came to class ready to work on a consistent basis. I always arrived on time, dressed out right away and went directly to my assigned group. I took all direction and followed all instructions.	Good Brought materials to class most days. Maybe forgot 1 item. Arrived on time most days. Dressed out in a regular amount of time. I might have forgot my shoes or dance materials a day or two.	Needs Improvement Forgot materials such as notebook/notes, music, or shoes more than once. Arrived at class late more than one time. Did not have or put on dance shoes. Did not cooperate most of dance assignment.
Classwork/Homework	Excellent I completed all CW and HW. I met all choreographic goals. I took all notes in my notebook during classroom lessons. I followed all the guidelines and criteria for each assignment. I turned my work in on time. I took pride in my work so that it was neat and organized.	Good I completed my CW and HW but may not have met all the criteria. I met most of my choreographic goals. I have most of the notes from classroom lessons in my notebook. I have handed in work late a couple of times and have forgotten to do my homework.	Needs Improvement I did not complete my CW and HW as directed. I did not meet any of the choreographic goals. I am missing notes from classroom lessons. I often forget my homework.
Behavior/Effort	Excellent I was on task nearly all of the time. I gave 100% in effort and tried my hardest. I worked well with other students. The teacher did not have to talk to me at all about my behavior.	Good I was off-task occasionally. I put forth strong effort and worked fairly hard. I worked well with other students. The teacher had to talk to me about my behavior once.	Needs Improvement I was off-task a lot I didn't try very hard. I didn't work well with other students. The teacher had to talk to me about my behavior more than once in this class.
