Preparedness/Rehearsals Did you come to class ready to work? Did you follow direction and listen?	Excellent I came to class ready to work on a consistent basis. I always arrived on time, participated right away and went directly to my assigned group. I took all direction and followed all instructions.	Good Most all days I was cooperative and did not disturb others. I learned the steps and followed directions.	Needs Improvement I arrived at class late more than one time. Did not have any effort into exercises and Did not cooperate most of dance assignment.
Classwork Participation Did you complete your work? Does it meet the requirements? Is your work ready when you come into the classroom?	Excellent I completed all CW . I met all participation goals. I took pride in my performance and class work.	Good I completed my CW but may not have met all the criteria. I met most of my performance goals. I did some "sitting days"	Needs Improvement I did not complete my CW I did not meet any of the performance goals. I am missing participation points most days because I sit or use my phone.
Behavior/Effort How did you work with others? Did you give 100%?	Excellent I was on task nearly all of the time. I gave 100% in effort and tried my hardest. I worked well with other students.	Good I was off-task occasionally. I put forth strong effort and worked fairly hard. I worked well with other students.	Needs Improvement I was off-task a lot. I didn't try very hard. I didn't work well with other students. The teacher had to talk to me about my behavior more than once in this class.
