Preparedness/Rehearsals Did you come to class ready to work? Did you follow direction and listen?	Excellent I came to class ready to work on a consistent basis. I was dressed out and prepared to learn. I took direction and followed all instructions. I was able to perform the tap combinations very well.	Good I came to class ready to work for most of the class. I was dressed out except for one item. I listened and took most of the directions. I was able to perform the tap combinations pretty well.	Needs Improvement I was not prepared for class. I was not dressed out and was distracted and did not listen to the instructions. I did not learn the tap combinations that were taught .
Personal choreography Did you work well independently orwith your partner to develop a tap combination using 6 of the skills we have covered during our tap unit.	Excellent I worked independently or with my partner(s) to develop a tap combination using 6 of the tap skills we have covered during our tap dance unit. The combination was original and creative. I/We performed the movement consistently each time we rehearsed.	Good I worked alone or with my partner(s) to create a combination using 6 of the tap skills covered during the unit. I/We did not spend much time and the movement is not very original or creative.	Needs Improvement I was unable to work productively to create a tap combination using 6 of the skills learned and practiced during our tap dance unit.
Behavior/Effort Were you focused during class and practicing the skills to the best of your ability?	Excellent I was on task nearly all of the time. I gave 100% in effort and tried my hardest. I worked well with other students. The teacher did not have to talk to me at all about my behavior.	Good I was off-task occasionally. I put forth strong effort and worked fairly hard. I worked well with other students.	Needs Improvement I was off-task a lot I didn't try very hard. I didn't work well with other students. The teacher had to talk to me about my behavior more than once in this class.
