Does not introduce self to patient, explain role in the clinic, ask the patient how they wish to be addressed , nor does student seek consent from patient before beginning interview

Student introduces self to patient, but fails to address one of the following: Give his name, ask patient how they with to be addressed, explain role in the clinic, or ask for patients consent before beginning.

Student gives patient name to patient, explains role in clinic, asks how patient wishes to be addressed and asks for consent before beginning interview.

Student does not ask appropriate questions or questioning seems to be chaotic. Intern does not begin with open ended questions and end with closed ended questioning.

Student asks appropriate open to closed ended questions, but failed to pick up on one or two points where they could have gone deeper. Questioning was somewhat ordered but not polished.

Student questioning is logical and ordered Student asks appropriate open to closed ended questions and picks up on minor details. Questioning and flow seem polished and fluid.

Student fails to summarize interview, or does so in such a manner that the summary is full of errors and seems pointless.

Student summarizes interview, but needs improvement, fails to mention something the patient told them that may be of consequence; or fails to ask patient if there is anything else they forgot to mention, or would like to add.

Student executes a near perfect summary of the interview, effectively covers all points mentioned by the patient. Asks patient if they have anything else that they would like to mention or add.
