Guided Community Assessment

The purpose is to understand the impact of the community setting upon the well-being of community members. The goals are to:1. Gain an awareness of the influence of the community and its resources on the health of the community; and 2. Continue to develop skills in observation and community assessment. For the purposes of this assignment, you will rely primarily on observation and information about your community and population that is available electronically.

Observation of Community 25%	Excellent The observation is thorough and all subsections in the table are addressed. Very descriptive, well-written and all directions followed.	Good Observations of subsections are stated or addressed but, lack detail. The community is primarily described.	Fair Observations in some subsections are addressed, a clear picture of the community is not accomplished.	Poor Observations of subsections are vague and non-descriptive.
Data on your community 25%	Excellent All census data is provided for each subsection with references. If there is a subsection that is not relevant, description about why it is not considered relevant.	Good At least 7 sections are described using referenced data.	Fair At least 5 sections are completed using data.	Poor Less than 5 sections or inaccurate information.
Synthesis Statement 25%	Excellent Synthesizing sources by pulling them together into some kind of harmony. Accurately reporting information from the sources using different phrases and sentences; organized in such a way to immediately see where the information from the sources overlap; makes sense of the sources and helps the reader understand them in greater depth. Demonstrates a link between observation and data. More than one sentence per subsection.	Good Condensed sentence for each subsection linking observation and data in a way that helps the reader to understand the sources in greater depth.	Fair Condensed sentence for at least 5 subsections linking observation and data.	Poor Simplistic statement regurgitating the observation and data.
Photo Mapping Activity 25%	Excellent One to two page summary in paragraph form of six photos that includes what you saw and key conclusions your group made about the community.	Good One page summary of six photos that includes what you saw and concluded about the community.	Fair Less than one page summary of some photos with vague conclusions	Poor Summary of photos.
