Formal Assessment Students will read a text and answer/identify the who, what, where, when, why, and how in a text. The student will be able to ask their own who, what, where, when, why, or how question about a text and answer the question with details from the text.
	Does Not Meet 1 pts	Meets 2 pts	Exceeds 3 pts
Demonstrate Understanding	Does Not Meet Student was able to find and analyze three or less of the elements in the story.	Meets Student was able to find and analyze about four or five of the elements in the story.	Exceeds Students can find and analyze about all six elements in the text (who, what, where, when, why, and how) to demonstrate their understanding.
ANSWER Questions	Does Not Meet Student was able to answer questions about three or less of the elements in the story.	Meets Student was able to answer questions about four or five of the elements in the story.	Exceeds Students can answer questions about all six elements in the text (who, what, where, when, why, and how)
ASK Questions	Does Not Meet Student was unable to ask a who, what, where, why, when, or how question about the text OR asked a question that was irrelevant to the key details of the text.	Meets Student was able to ask a who, what, where, why, when, or how question about the text.	Exceeds Student was able to ask a who, what, where, why, when, or how question about the text to gather more information about a text and find the answer.
