Student was on-task & worked productively during classes. Student met the highest level of development & achieved self-discipline, demonstrated initiative & responsibility.
Good
Student was On-Task for the majority of time and worked productively. I met the majority of the behavior expectations, but felt that I had external motivation to keep me on task.
Fair
Student was On-task and productive part of the time. There was more than one time when I had level C behavior and had to complete an essay to explain my behavior.
Poor
Student was NOT On-Task and productive throughout the class period. Student demonstrated either level C or D behavior and had to complete an essay or self-diagnostic referral.
Participation
Excellent
Student contributed to class democracy by OFFERING ideas and suggestions and by helping others, or asking others for help during the class.
Good
Student contributed to class democracy by offering ideas and suggestions. Student complied with Level C expectations of classroom.
Fair
Student occasionally contributed to classroom democracy, by offering ideas and suggestions and by helping others, but had to be reminded to stay on Level C.
Poor
Student DID NOT contribute to activity by offering ideas and suggestions. Due to this level B or A behavior, student had to complete behavioral paperwork.
Listening Skills
Excellent
Student LISTENED when others talked.
Made responses appropriately.
Good
Student listened when others talked.
Fair
Student often DID NOT listen when others talked. Student behavior was on level B.
Poor
Student DID NOT listen and was disruptive when others talked.

Student SHOWED responsibility and respect and did NOT display disruptive behavior during activity.

Student MOSTLY showed responsibility and respect and RARELY displayed disruptive behavior during activity.

Student OCCASIONALLY showed responsibility and respect and OCCASIONALLY displayed disruptive behavior during activity.

Student DID NOT show responsibility and respect during activity.

Student used the computer at the appropriate times and used software appropriate for activity.
