Utilizes more then 3 phisical assessment to develop a data set for care plan. Consistently provides subjective data in quotes and provides clear, specific, objective data. Data is consistently relevant and clustered in a coherent manner. Data clearly supports nursing diagnosis.
Nursing Diagnosis

The students will develop 3 nursing diagnosis. One of the nursing care plan can be related to psycho-social issues or patient education. Each Diagnosis with have all 3 parts, the NANDA nursing diagnosis, the related factor, and the evidence. The only exception is “risk for” nursing diagnosis which lack evidence.

Unsatisfactory: Below Expectations

None of the nursing diagnosises are clearly supported by data or they are not complete. Nursing diagnosis does not reflect an accurate clinical judgment. developed less then 3 nursing diagnosis
Satisfactory: Meets Expectations

Two out of the three nursing diagnosises that are clearly supported by assessment data and reflects an accurate clinical judgment.

Goals/Outcomes

They will give a short term goal that should be met by the end of their interaction with the client, and they will give a long term goal that can be met by discharge. All goals will be “SMART”.

Does not identify at least 2 outcomes for each nursing diagnosis. Outcomes are unrelated to the nursing diagnosis. Outcomes are consistently not client-centered, measurable or achievable by target dates.
Satisfactory: Meets Expectations

Identifies 2 outcomes for each nursing diagnosis. Outcomes are client-centered, measurable, and achievable with specific target dates.
Exceeds Expectations

Identifies 2 or more outcomes that clearly reflect a change in the nursing diagnosis (in a positive direction). Outcomes are consistently client-centered, measurable and achievable, with specific target dates.
Nursing Plan/Implementation

The students will give interventions to meet each short term and long term goal.

Unsatisfactory: Below Expectations

Does not identify at least 1-2 interventions for each outcome. Interventions are unclear and/or do not clearly focus on the etiology of the nursing diagnosis or convey how the nurse will help the client meet outcomes.
Satisfactory: Meets Expectations

Identifies 1-2 specific nursing interventions for each outcome. Interventions are individualized to the client. Interventions focus appropriately on the etiology of the nursing diagnosis and reflect how the nurse will assist the client in meeting the outcomes.
Exceeds Expectations

Identifies 2 or more specific interventions for each outcome. Interventions are clearly linked to outcomes and focus specifically on the etiology of the nursing diagnosis. Oucomes clearly reflect what the nurse will do to help the client meet the outcomes.
Evaluation

The student will evaluate each nursing diagnosis as if the patient met the goal. The students would give evidence as to how the goal was met.

Unsatisfactory: Below Expectations

Does not provide clear data to evaluate outcomes or does not state specifically how outcomes would be evaluated (if no opportunity to evaluate). Does not state whether each outcome was met, partially met, or unmet. Does not address revisions needed in careplan.
Satisfactory: Meets Expectations

Provides clear data to evaluate each outcome (or clearly states how each outcome would be evaluated). Makes accurate judgments about each outcome as met, partially met, or unmet. Identifies revisions needed in careplan.
Exceeds Expectations

Consistently provides clear data to evaluate each outcome (or states clearly how each outcome would be evaluated). Makes accurate judgments about each outcome as met, partially met, or unmet. Discusses revisions needed in careplan and rationale for revisions.
