	Exceptional 5 pts	Satisfactory 4 pts	Unacceptable 0 pts
Timeliness 25 % Each encounter is entered into Typhon in a timely manner.	Exceptional Case logs are submitted on time.	Satisfactory Some case logs are submitted late.	Unacceptable Case logs are not submitted.
Accurateness 25 % Each encounter provides accurate CPT and ICD-9 codes.	Exceptional The student provides consistent accurate CPT and ICD-9 codes for the type of visit, PMH, procedures performed, diagnostic testing, and any other pertinent information.	Satisfactory The student provides, for the most part, consistent accurate CPT and ICD-9 codes for the type of visit, PMH, procedures performed, diagnostic testing, and any other pertinent information; however, there are occasional errors.	Unacceptable The student has multiple errors in CPT and/or ICD-9 codes for the type of visit, PMH, procedures performed, diagnostic testing, and any other pertinent information.
Completeness 25 % Each encounter provides a complete picture of all areas addressed during the visit (i.e. social issues, medications, competencies).	Exceptional The student provides complete case logs.	Satisfactory The student, for the most part, provides complete case logs.	Unacceptable The student has multiple incomplete case logs.
Accountability 25 % The student accounts for how his/her time is spent throughout the day for the week.	Exceptional The student accurately accounts for their shift time during the course of the week.	Satisfactory The student, for the most part, provides an accurate account of their shift time during the course of the week.	Unacceptable The student provides poor account for their shift time during the course of the week.
