The student never provides complete case logs. This includes social problems addressed, procedures/skills, medications, etc. when applicable.
Accountability

The student meets the minimum requirement of hours for the rotation and provides correct explanations for days that are missed. The student also accounts for how his/her time is spent throughout the day.

Exceptional

The student is on track to meet the required minimum hours for the rotation and provides excellent account for their time during the course of the week. Total shift time and total allocated time vary by less than 5 hours for the week.
Satisfactory

The student is on track to meet the required minimum hours for the rotation and provides adequate account for their time during the course of the week, and/or total shift time and total allocated time vary by 5-10 hours for the week.
Marginally Acceptable

The student is on track to meet the required minimum hours for the rotation but provides an unsatisfactory account for their time during the course of the week, and/or total shift time and total allocated time vary by >10 hours for the week.
Unacceptable

The student is not on track to meet the required minimum hours for the rotation and consistently provides poor account for their time during the course of the week; in addition the total shift time and total allocated time vary by >10 hours for the week.
