Monday	Attendance 5pts on-time to class and is in class and seat when bell rings	Supplies 5pts student has all supplies for class that day 4pts student forgot one supply or borrows from teacher	Self-Dicsipline 5pts if student is behaved and following class procedures and stays on task 4pts if student has to be reminded once to adhere to procedures or task in class	Class Discussion 5pts if student participates fully in class discussions or assigned tasks throughout class time 4pts if student mostly participates in class discussions or tasks	Classwork 5pts if student turns in classwork or assignments on time 4pts if student turns in classwork or assignments 1 day late
Tuesday	Attendance 4pts if tardy to class shortly after bell rings 3pts if tardy 5-10min after bell rings 2pts if tardy 10-15min after bell rings	Supplies 3pts if student is missing 2 or more supplies 2pts if students damage or misuse supplies	Self-Dicsipline 3pts if student has to be reminded twice to adhere to procedures or task in class 2pts if student three or more times to be redirected in class and is disruption to the class	Class Discussion 3pts if student soemtimes participates in class discussions or assigned tasks during class 2pts if student partially participates in class discussions or assigned tasks during class	Classwork 3pts if student turns in classwork or assignments 2 days late 2pts if student turns in classwork or assignments 3 days late
