Clinical Preparation

Student is observed off task, not focused on the exercise. The task is incomplete of 1 assigned duty. Begins exercise within 5 minutes after entering the classroom.

Unacceptable

Student is observed off task, not focused on the exercise. The task is incomplete of 3 or more assigned duty. Begins exercise within 10 minutes after entering the classroom.
Dress Code

Uniform neat and clean, ID present, closed-toe shoes, watch, 1 pair small post style earrings to ears (optional for females only), hair neat and pulled back, short nails. See course uniform guidelines.

Unacceptable

Components of professional behavior are not met: unprofessional behavior, lack of communication skills, and/or poor attitude.
Student does not store cell phone appropriately, with established class rules.
Communication Skills

Interacts appropriately with instructor, and other students. Reports abnormal findings to instructor.

Acceptable

Interacts appropriately with classmates and instructor at all times. Reports any abnormal findings to instructor. Asks questions prior to performing task if unsure.

50% of the time interacts appropriately with classmates and instructor. May report incidental abnormal findings. May ask questions prior to performing task if unsure.

25% of the time interacts appropriately with classmates and instructor. Does not report abnormal findings. Rarely will ask questions, even if unsure with performing task.

No appropriate interaction with classmates or instructor. Does not communicate needs to instructor. Does not ask questions with performing tasks if unsure.
Rules and Safety

Follows safety, infection control rules. Ensures safety for other students; makes appropriate safety/infection control judgements.

Acceptable

Follows all rules and guidelines at all times, asks instructor prior to performing task if unsure how to perform.

One violation of safety/rule guidelines.
Below Average

Two violations of safety /rule guidelines
Unacceptable

Violation of safety and/or confidentiality guidelines and rules. Student performs tasks in unsafe manner.
