	Poor 1 pts	Fair 2 pts	Good 3 pts
Segmentation	Poor Unable to identify the number of sounds (phonemes) in words, with at least 60% accuracy.	Fair Correctly identifies the number of sounds (phonemes) in words with at least 60% accuracy.	Good Correctly identifies the number of sounds (phonemes) in words with 100%accuracy.
Isolation	Poor Unable to identify/ name sounds at the beginning, middle, or end of words with at least 60% accuracy.	Fair Correctly identifies isolated sounds within words with at least 60% accuracy, but consistently struggles in one or more areas (ex: ending sounds).	Good Correctly identifies beginning, middle, and ending sounds of spoken words with 100% accuracy.
Deletion	Poor Unable to orally delete phonemes from beginning, middle, or ends of words with at least 60% accuracy.	Fair Correctly deletes isolated sounds within words with at least 60% accuracy, but consistently struggles in one or more areas (ex: ending sounds).	Good Correctly deletes beginning, middle, and ending sounds of spoken words with 100% accuracy.
Addition	Poor Unable to orally add phonemes to the beginning, middle, or ends of words with at least 60% accuracy.	Fair Correctly adds isolated sounds to words with at least 60% accuracy, but consistently struggles in one or more areas (ex: ending sounds).	Good Correctly adds beginning, middle, and ending sounds to spoken words with 100% accuracy.
