Cover Page Name, title of project, course name, teacher's name, and date.	None Portfolio contains no cover page at all.	Poor Cover page is not typed and only contains one or two of the required elements.	Fair Cover page is typed and contains only two to three of the required elements.	Good Cover page is typed with all but one of the required elements, but with errors.	Excellent Cover page is typed, with no errors and all required elements are listed.
