Explain Procedure to Patient Perform hand hygiene. Identify the pt (2 identifiers). Introduce yourself. Explain the procedure. Proper pt position (legs uncrossed, feet on floor, arm at level of heart).	Unsatisfactory Did not perform hand hygiene. Did not identify the patient. Did not introduce. Did not explain the procedure. Patient not in proper position.	Unsatisfactory Missed some of the steps	Pass Explained procedure to the patient. Did not miss any of the steps.
Pulse & Apply Cuff Find pt radial pulse. Count the pulse for 1 full minute. Apply the proper size BP cuff. Determine how high cuff should be pumped up for BP reading.	Unsatisfactory Had difficulty finding the pulse at the wrist, but finally was able to get this correct. Did not choose correct cuff size. Was not able to determine how high to pump the cuff. Needed several tries to do this and had to adjust the tightness.	Unsatisfactory Found the pulse at the wrist after several minutes. Did some fumbling with the cuff, but was able to apply correctly. Did not determine, or remember how high to pump the cuff.	Pass Found the pulse quickly at the wrist and took it correct the first attempt. Palpated the pulse point in the arm and applied the appropriate size cuff properly with the gauge in the front at the pulse point. Was not too lose or too tight.
Cuff Inflation & Blood Pressure Inflate cuff to pre-determined amount. Competently adjust the valve to inflate and deflate the cuff.	Unsatisfactory Needed several attempts to get the cuff inflated and left it up too long causing some pain. Could not read the gauge properly.	Unsatisfactory Took several attempts to inflate and deflate the cuff properly. Struggled with the valve and reading the gauge.	Pass Handled the valve correctly and inflated the cuff just above where it needed to be. Let the air out slowly but not painfully slow. Was able to read the gauge without hesitation.
Obtain Reading Read the gauge with confidence. Quickly deflated the cuff after getting BP reading. Removed the cuff and informed the pt of the BP reading.	Unsatisfactory Obtained the correct reading on the second try but demonstrated little confidence. Did not deflate the cuff quickly. Did not speak to the patient.	Unsatisfactory Obtained the correct reading on the second try. Left the cuff pumped up too long. Left the cuff on too long. Did not inform the patient of the reading.	Pass Obtained the correct reading on the first try. Quickly deflated the cuff and removed it after obtaining the BP reading. Confidently informed the patient of the BP reading.
