This is a rubric for support providers (mentors) to self evaluate the level of PT (teacher) observation to ensure multiple means of formative assessment.

Use of VoiceThread	Minimal Response difficult to view and difficult to find on the thread.	Moderate Response slightly disorganized for easy viewing.	Complete All information presented in a logical and easy to view/read manner (summary and responses)
Use of Screen Capture	Minimal No description of what might be done differently and no probing questions to the PT.	Moderate Partial description of needs of students being met and/or why included and may or may not include questions for the PT.	Complete Purpose of screencast is clearly stated. Feedback is detailed to PT including student needs and reflective questions.
Use of YouTube	Minimal No recommendations were made to improve instruction.	Moderate Little connection was made between the teacher observation and reflective feedback.	Complete Several connections were made and recommendations for improving or continuing instruction as well as reflective feedback given.
