TR Department Quality Improvement Project 
The intern will have the opportunity to review the QI process at Waverly, and participate in development of a QI project. The student will write up and submit the QI report, and speak at the QI meeting about the project. Student will also be asked to complete a QI survey of the department (including chart audits and group facilitation audits) and present findings at a TR staff meeting.

25 %

Departmental audit following state regulations for skilled nursing recreation departments is filled out.

Poor

Does not complete the majority of the audit, or is completed but with many inaccuracies in information.
Fair

Mostly completes the departmental audit, or completes the audit but with some inaccuracies of developmental needs
Excellent

Completes the entire departmental audit accurately, identifying areas of need or opportunities for development.
Chart Audits
25 %

5 chart audits are completed, ensuring completion of all TR documentation on each unit.

Poor

Does not complete the majority of the audits, or they are completed but with many inaccuracies in most or all areas.
Fair

Completes most of the chart audits, or completes all, but does so inaccurately in some areas.
Excellent

Completes all 5 chart audits accurately, identifying areas of need or opportunities for development.
Group facilitation audits
25 %

5 group facilitation audits are completed randomly on different TR staff members, ensuring proper facilitation of programming.

Poor

Does not complete majority or all of group audits, or they are completed with inaccuracies in information, scoring and recommendations.
Fair

Completes most of the group facilitation audits, or shows some inaccuracy in representing what occurring in the group. Scoring is correct, but recommendations are not necessarily appropriate.
Excellent

Completes all 5 group facilitation audits, accurately representing what occurred during the groups. Documents are scored correctly and recommendations are made appropriately.
Presentation of QI audit findings
10 %

A report of QI audit findings is represented to TR staff at a staff meeting.

Poor

Information is not presented at a TR staff meeting.
Fair

Information obtained from the QI audit is presented at a TR staff meeting, bu may not be clear or identify all areas of development.
Excellent

Information obtained from the QI audit is presented (5 minutes or less) at a TR staff meeting in a clear and concise way, identifying areas of development when applicable.
Input for future QI project
10 %

Give input into future QI project for the department, using information obtained from audits and knowledge of field.

Poor

Student does not give input for a QI project.
Fair

Student gives input into a departmental QI project, but input is not based on needs of the department, or may not work with the department.
Excellent

Student uses information obtained from audit process and time spend in the department to input creative and helpful ideas for a quarterly QI project for the department.
Relation to standards of practice
5 %

Demonstrate a knowledge of how the ATRA Standards of Practice relate to the TR Department Audits, and how the SoP can be used as an audit tool.

Poor

Student is not able to verbalize how the SoP could be used as an audit tool or why it is important to adhere to the SoP while running the department.
Fair

Student is able to verbalize either how the SoP could be used as an audit tool, OR the importance of adhering to the SoP while running the department.
Excellent

Student is able to verbalize to supervisor how the Standards of Practice could be used as an audit tool, as well as the importance of adhering to the SoP while running the department.
