Medication Administration

Learner identifies indication. Identifies why the patient is prescribed the medication.	Unsatisfactory The learner requires prompting to consider the indication.	Needs development The learner considers the indication but is unable to identify how this can be determined. Eg. Handover Patients notes/history Patient	Satisfactory The learner correctly identifies the indication for the medication.
Learner washes hands Washes hands according to NSW MoH infection control guidelines around preparation and time.	Unsatisfactory The learner requires prompting to wash hands	Needs development The learner washes hands but does not adhere to the NSW MoH infection control guidelines.	Satisfactory The learner washes hands in accordance to the NSW MoH infection control guidelines.
Learner abides by QUM principles. Quality Use of Medication principles including: Legal responsibilities. Calculation of accurate drug dosages. Identifies understanding of pharmacology prior to administration.	Unsatisfactory The learner requires prompting to consider one or more QUM princples.	Needs development The learner considers only one or two of the identified QUM principles.	Satisfactory Three or more QUM principles are adhered to in the administration of the medication.
Gathers equipment. Medication chart Medication trolley Fresh water and a glass.	Unsatisfactory The learner requires prompting to gather any equipment.	Needs development The learner fails to gather all necessary equipment.	Satisfactory All the necessary equipment is gathered.
Therapeutic communication The learner introduces self to patient and provides an explanation of the procedure. The learner discusses effects and adverse effects of the medication with the patient.	Unsatisfactory There is no evidence of therapeutic communication with the patient.	Needs development The learner attempts to communicate with the patient but requires prompting to introduce themselves, explain the procedure and /or, discuss the medication effects and adverse effects.	Satisfactory The learner shows evidence of therapeutic communication, introducing themselves, explaining the procedure and discussing the effects and adverse effects of the medication.
Uses Six Rights Right: Patient Drug Dose Route Time Documentation.	Unsatisfactory The learner requires prompting to consider the "six rights" of medication administration.	Needs development The learner is unable to identify and adhere to all "six rights" of medication administration.	Satisfactory All "six rights" of medication administration are identified and followed.
Checks and prepares medication. Medication checked three times prior to giving. Medication dispensed into medication container using a non touch technique.	Unsatisfactory There is no evidence of the medication being checked. Medication is dispensed without consideration of a non touch technique.	Needs development The learner checks the medication but less than three times prior to giving and/or the learner attempts but fails to use a non touch technique to dispense the medication.	Satisfactory The learner checks the medication three times prior to administaration and employs a non touch technique to dispense the medication.
Assists patient to take medication. Uses problem solving as required. Re-positions patient if necessary. Holds water and/ or medication if required. Assists patient to place medication in mouth as required.	Unsatisfactory Learner does not stay with patient to assist with taking the medication.	Needs development Learner stays with the patient but does not employ problem solving to assist the patient to ingest the medication as required.	Satisfactory Learner stays with the patient and employs problem solving to assist the patient ingest the medication as required.
Documents relevant information. Documents correctly in the medication chart following the patient ingesting the medication and in the patient notes as required	Unsatisfactory Fails to document in medication chart and/or patient notes.	Needs development Documents in medication chart but does so either before the patient ingests the medication or documents incorrectly in the medication chart and/or patient notes.	Satisfactory Documents correctly in the medication chart and/or patient notes, directly following the patient ingesting the medication.
Cleans restocks and returns trolley Wipes down the medication trolley. Restocks medications as required. Locks trolley on accordance to NSW MoH guidelines and returns to appropriate place.	Unsatisfactory Fails to clean and restock and return trolley to appropriate place OR fails to lock the trolley.	Needs development Fails to clean or restock or return locked trolley to appropriate place.	Satisfactory Cleans, restocks and returns locked trolley to appropriate place.
