Drug Information Booklet
Students will create a drug information booklet consisting of Cover,index page, brand/generic with a variety of names and forms of medication (ex:capsule, caplet, injection) with dosage strengths, list the following:indication, usage, missed dose, drug adverse reaction/interaction/warnings, side effects, other indications, medication storage, manufacturer of medication, statistics, and reference page. Extra points given if student lists website and phone number for patient assistant program and how to access this information. Student must create a booklet to catch the attention of fellow students with pictures of medication and create informative and accurate information.

All words are spelled correctly. Grammar and sentence structure are correct. The booklet contains a table of contents, glossary and bibliography. 2 or more references are used.

Some words are misspelled. Only 2 format componenets are used. Wording is less than clear at times. Only 1 reference is used.
Fair
1 format component is used. Spelling mistakes appear often. No references are stated.
Poor
No format components are used. Text is unreadable. Spelling misstakes are overabundant.
Editting
Excellent
Student goes to great length to perfect their booklet.
Good
Student uses some of the recommendations to change their booklet.
Fair
Students do some editting. Many misstakes remain.
Poor
Students do not edit their booklet.
Pictures
Excellent
Student shows excellent variety of pictures consisting with one or more of each: medication bottle, forms of medication, statistics and molecular structure.
