Postitioning of Patient Student provides for privacy by closing doors and pulling screening curtian. Perineal Care Wet wash cloths, make mitted cloth, apply soap, retract foreskin if needed, clean tip of penis from meatus using circular motion , repeat until clean using a different area of the cloth each stroke. rinse with clean cloth using same technique. pat dry with towel .then clean the shaft of the penis using a downward strok using a different area of the wash cloth for each stroke	Poor unable to complete without assistance of more that 1 time from instructor or completes in an unsafe manner as noted below: __________________	Fair completed but student required 1 assist from instructor. Perineal Care .rinse in the same way and pat dry. fold the blanket back, lower the legs and turn to the side away from you. apply soap to mitted wash cloth, clean the rectal area from scrotum to the anus using the same technique as before, rinse and pat dry.	Poor	Fair	Good
Catheter Care After providing perineal care grasp the catheter tube at the insertion site. cleanse downward about 4 inches away from the body using a different section of the cloth at least 3-4 times or as needed til clean. rinse and dry using the previous technique. Post Procedure Remove any soiled linens or moisture barrier products and dispose of properly, position patient for comfort, perform safety check of the room, remove gloves and wash hands. make sure call light and personal objects are within reach. thank the patient.	Poor unable to complete without assistance of more that 1 time from instructor or completes in an unsafe manner as noted below: __________________	Fair completed but student required 1 assist from instructor. __________________	Good all items completed without assist of instructor.
