Completed Report	Novice Report is not complete as well as time log. Supervision signatures are there but the report is blank.	Developing Student leaves several items blank and has notes jotted down; No evidence the student has requested help from site supervisor or internship faculty.	Good No blanks, student makes an attempt to complete all areas of the daily report;	Proficient No blanks, student was able to link learning with classroom objectives from the program. Student is able to access information to answer questions, form opinions, or solve problems.
Relative Tasks	Novice Tasks are vague or not listed.	Developing Student list the same task on all notes with no variety of skill learning.	Good Students list tasks assigned; no integration or impact of learning or new skills.	Proficient Students list tasks completed showing progression of learning and/or integration of new skills.
Ethics	Novice Ethical code response is left blank or states NA or None	Developing Student uses the same ethical code, with little motivation to explore the information further.	Good Student links an ethical code, but does not fully explain or link the ethic to an activity. Same sentence is used throughout the note,	Good Student gives a summary comment, but does not have a well thought out plan to incorporate their own as well as others assumptions, beliefs, and opinions.	Proficient Student is able to evaluate one's own and other's assumptions, beliefs, and opinions.
Completed Report	Novice Report is not complete.	Developing Student does not present a clear able to read document. Some signatures are missing. Documentation is not approved due to missing signatures from supervisor verifying the student was present.	Good Report is readable, some professional language is used, one to two spelling errors. Some problems with dates and signatures.	Proficient Report is easily readable, free of spelling errors; appropriate professional language is used; date, time and signatures match the log file.
