iRubric: Oral Hygiene for the Unconscious Patient rubric

*Wash hands for at least 15 seconds.
*Have bed flat (if this is not restricted.) Position patient on their side with head well to the side.

*Washed hands less than 15 seconds.
*Attempted to position patient but was not accurate (i.e. head was not to the side enough)

*Did not wash hands.
*Did not position patient correctly. Patient is at risk for aspiration in the position he/she is in.

*Use sponge swab moistened with mouthwash (if not restricted). Squeeze out excess mouthwash over basin, before putting in patient's mouth

*Did not add cleaning agent to spoinge swab.
*Did not squeeze out swab

*Use new swab with water (squeeze out excess before placing in patient's mouth) to swab around mouth to rinse out the mouthwash.

*Did not rinse mouth with new swab moistened with water.
*Did not squeeze excess water out of sponge before putting in patient's mouth.
