Nursing Diagnosis	Poor Non approved nursing diagnosis regardless of other information	Fair Approved nursing diagnosis Missing or unrelated "related to" or "as evidenced by" if applicable	Good Approved nursing diagnosis with accurate information regarding what the diagnosis is related to and the "as evidenced by" if applicable. Bonus point if you have this all !!
Goal	Poor Goal is missing 2 or more of the following elements (measurable,realistic,related)	Fair Goal is missing one of the follow elements (measurable,realistic,related)	Good Goal is measurable, realistic and is directly related to solving the selected nursing diagnosis
