Supine Position
Level 4
Lifts head as if wanting to get up. Bangs and shakes objects.
Level 3
Rolls from back to stomach. Takes feet to mouth. Transfers objects from one hand to the other. Reaches with one hand.
Level 2
Holds head steady in midline. No head lag when pulled to sitting. Rolls from back to side. Kicks feet vigorously in air for play. Grasps foot with hand.
Level 1
Rolls part way to side. Turns head in both directions. Rolls from side to back. Bends and straightens arms and legs.
Level 0
Student can not perform skills from supine position.
Prone Position
Level 4
Changes from creeping (does not use arms and legs in opposition) to sitting position. Can perform all tasks listed for prone position.
Level 3
Supports most of weight on hands. Crawls, dragging body. Creeps on hands or knees or scoots on buttocks.
Level 2
Reaches while supported on one elbow. Rolls to back. Pivots turns on stomach.
Level 1
Lifts head up to 90 degrees. Raises chest with elbow and forearm support.
Level 0
Student can not perform skills from prone position.
Sitting Position
Level 4
Sits with enough balance and support to free hands for an activity, such as pat-a-cake. Can perform all tasks listed for sitting positions.
Level 3
Sits erect and unsupported for five minutes. Pivots/Turns on buttocks. Goes from sitting to creeping position without losing balance.
Level 2
Sits when supported. Sits briefly unsupported, may lean forward on hands. Sits steadily with little risk of overbalancing.
Level 1
Sits with head set forward with some control, but also with some bobbing. Steadies head but does not hold it erect. Attempts to right self when tilted.
Level 0
Student can not perform skills from sitting position.
Standing Position
Level 4
Gets to standing position without support. Walks well with hands low (at side) and rarely falls. Can perform all tasks listed for standing position
Level 3
Pulls to standing position, walks with one hand held when led. Walks alone with hands high to aid balance.
Level 2
Supports full weight when balanced. Stands holding on to an object for support. Walks with both hands held.
Level 1
Supports small fraction of weight briefly. Lifts one foot when held in supported standing position. Supports most but not all weight.
Level 0
Student can not perform skills from standing position.
