Student on Task	Did Not Participate Student did not participate in the class activity or required more than six prompts to stay on task.	Needs Improvement Student had limited participation in the lab activity.	Satisfactory Student participated in the lab activity from time-to-time, but did not put forth much effort.	Excellent The student participated in the lab activity the entire time and gave 100% effort. Stayed on task and gave 100% attention to the teacher.
Student Behavior	Did Not Participate Did not participate in the activity or behavior had to be corrected more than four times.	Needs Improvement Student had to be redirected three or more times for talking too loud and/or during direct instruction, disrupting others or other inappropriate behavior.	Satisfactory Student had to be redirected one to two times for talking too loud and/or during direct instruction, disrupting others or other inappropriate behavior.
