Chapter Activities Must have provided a list of the major chapter committees that he/she has chaired or held office.	Poor Nominee has chaired one chapter event	Fair Nominee has participated in chapter events at various levels.	Good Nominee only chaired or held office within the last year.	Excellent Nominee organized a major chapter events/functions. The activity was described in detail.
Support/Fund Raising Must have contributed at least $1,000 to the university or must have raised that amount, or more, for the university.	Poor Nominee assisted with chapter fundraising	Fair Nominee was chairperson and motivated the chapter activities leading to a successful fund raising event	Good Nominee contributed to a fund raising event.	Excellent Documentation showing the nominee contributed at least $1,000 to the university or sponsored/supported a fund raising at least $1,000 to support the alumni association.
Personal Accomplishments Nominee must have made notable civic, political, cultural or other signficant achievements (explain all).	Poor Nominee provided supporting documents for chapter recognition only.	Fair Nominee provided supporting documents for chapter and church recognition.	Good Nominee provided supporting documents for civic, political and cultural events.	Excellent Information provided must have occurred within the current fiscal year. Explain in one page or less.
