Identification Student will be able to accurately identify three ways that common communicable diseases (cold, flu, etc.) can be spread.	Unable to Demonstrate Student is not able to identify any of the ways in which common communicable diseases are spread.	Needs More Work Student is able to identify some of the ways in which common communicable diseases are spread, but needs prompting/assistance from the classroom teacher more than 50% of the time.	Proficient Student is able to identify three of the ways in which common communicable diseases are spread, with prompting/assistance from the classroom teacher less than 50% of the time.	Mastered Student is able to identify three ways in which common communicable diseases are spread with no prompting/assistance from the classroom teacher.
Prevention Students will be able to accurately identify three ways to prevent common communicable diseases.	Unable to Demonstrate Student is not able to identify any of the ways to prevent common communicable diseases. Needs More Work Student is able to display parts of the proper hand-washing techniques, but needs prompting/assistance from the classroom teacher more than 50% of the time.	Proficient Student is able to display proper hand-washing techniques, with prompting/assistance from the classroom teacher less than 50% of the time or Student is able to display proper hand-washing techniques with no prompting/assistance from the classroom teacher, but uses visual sequence for reference.	Mastered Student is able to display proper hand-washing techniques with no prompting/assistance from the classroom teacher, and without the use of a visual sequence.
