Documentation is accurate Assessment and documentation is accurately recorded in a timely manner	Non-performance Documentation is not completed correctly or completely lacking patient assessment requiring Instructor intervention	Below Expectations The MAR log is either not signed or initialed with patient assessment being incomplete or wrong	Basic The MAR log is signed and initialed outside the prescribe time frame with an incomplete patient assessment	Proficient The MAR is initialed and signed correctly but is outside the prescribed time frame with the approriate patient assessment	Distinguished The MAR is initialed and signed correctly in a timely manner that includes approriate patient assessment
Medications are secured Medications are handled and stored correctly per facility policy	Non-performance Medications are left unattended at the bedside requiring Instructor intervention	Below Expectations Not all medications are secured or stored according to faciltiy policy following administration requiring Instructor intervention	Basic Medication storage and securement are attempted by some components are omitted requiring Instructor intervention	Proficient The medications are stored and secured per faciltiy policy with student requiring prompting for explaniation of securement process	Distinguished The medications are stored and secured per faciltiy policy with learner correctly explaining securement process
