Cover Page	Poor Itemized list is out of order or does not include all work.	Fair Itemized list is not in order of activity number, but all work is included, or list is in order but all work is not included.	Good Itemized list in order of activity number, of all work presented, including worksheets, results pages, samples and model information.
Samples	Poor Includes less than half of the samples/models from each skill practiced or missing labels.	Fair Includes one-half or more of the samples/models with labels or includes all samples or models but all are not labeled.	Good Includes a sample/model from each skill practiced, labeled with a title and activity number.
Test Results Pages	Poor Does not include written corrections.	Fair Missing new score or not in order according to activity number but has written corrections.	Good Includes written corrections, includes new score, and are organized in order of activity number.
Web-sample	Poor Not submitted.	Fair Submitted but not in the correct format.	Good Submitted in the correct format.
