Identifies goals/s	Poor Student does not identify an academic, social/emotional and/or vocational goal.	Fair Student requires moderate assistance to identify his/her academic, social/emotional and/or vocational goal.	Good Student requires minimal assistance to identify his/ her academic, social/emotional and/or vocational goal.	Excellent Student requires little to no support to identify his/her academic, social/emotional and/or vocational goal.
Identifies action plan	Poor Student does not identify their action plan.	Fair Student requires moderate assistance to identify his/her action plan.
