Repeat 3 unrelated words Examiner instructs client to listen carefully, remember the words and repeat them back to examiner. Examiner then clearly says three words and has the client repeat them, ensuring they heard and registered the correct word. Examiner tells the client to remember these because the examiner is going to ask client to repeat them later.	Excellent Examiner performs task with no errors and does so efficiently and with confidence.	Good Examiner performs task with no errors.	Fair N/A	Poor Examiner fails to have client repeat words back OR Fails to instruct client to remember the words for later.
Clock Drawing Test (CDT) Examiner provides pencil and paper and asks client to draw a clock face with the numbers on it. After that is complete, examiner asks client to draw in the clock hands showing a specified time.	Excellent Examiner performs the 3 tasks with no errors and does so efficiently and with confidence.	Good Examiner performs the 3 tasks with no errors.	Fair Examiner fails to perform one of the 3 tasks: 1. provide pencil/paper 2. have client draw clock face with numbers 3. have client draw in a specified time	Poor Examiner does not administer CDT before asking client to repeat the words they were told to remember for later.

Score the test Give 1 point for each word recalled after clock drawing test (CDT). NEGATIVE screening for dementia: Score of 3 OR Score of 1 or 2 with normal CDT POSITIVE screening for dementia: Score of 1 or 2 with abnormal CDT OR Score of 0	Excellent Examiner performs task with no errors and does so efficiently and with confidence.	Good Examiner performs the task with no errors and correctly identifies the meaning of the score.	Fair Examiner assigns the correct score but is unsure of what it means (positive for dementia or negative for dementia).	Poor Examiner assigns the wrong score.
