1st position of gown Surgical Gown laid out on with inside of the gown facing the table with sleeves crossed in front.	Poor Gown was facing inside out.	Fair Gown is faced correctly but sleeves were not evenly crossed.	Good Gown is faced correctly and sleeves evenly crossed.
2nd Gown Fold Surgical gown sides are folded to center with armpits seams even with each other. The gown is folded over on itself (hotdog bun style) making sure gown is neat and crisp.	Poor Gown armpits seams are not even with each other. and gown is folded over on itself but lacks neat and crisp.	Fair Gown armpits seams are 50% even and gown is folder over on itself with 75% neat and crisp folds.	Good Gown armpit seams are even and gown is folded over on itself with crips, neat folds.
3rd Gown Fold Sides of gown are folded to center of gown with edges meeting in the middle. Make sure all strings are tucked in and gown is neat and crisp.	Poor Gown is folded to center but edges do not meet evenly in the middle, strings not tucked in and gown is not folded neat and crisp.	Fair Gown is folded to center but edges with 50% of the seam met in the middle, strings tucked in or partially tucked in. 75% of the gown is folded neat and crisp.	Good Gown is folded to center with 100% of the seam met in the middle, strings tucked in and 100% of the gown is folded neat and crisp.
4th Gown Fold Neck of gown is accordian folded to the hem of the gown in nice crips folds that do not exceed 5 folds with indicator strip placed in second fold.	Poor Did not start fold at neck or squarely at neck. The gown did not stay in a nice crisp fold and exceeded 5 folds. Indicator strip was forgotten	Fair Started fold squarely at neck. The 50% gown stayed in a nice crisp fold and did not exceeded 5 folds. Indicator strip was remembered.	Good Started fold squarely at neck. The 100% gown stayed in a nice crisp fold and did not exceeded 5 folds. Indicator strip was remembered.
Hand Towel Hand towel is accordion folded lengthwise first with no more than 4 folds. It is then accordion folded from the top down with no more than 4 folds.	Poor Hand towel was not accordion folded lengthwise. Exceeded more than 4 folds. it was accordion folded from top down with more than 4 folds. Folds were not crips and neat.	Fair Hand towel was accordion folded lengthwise. Did no exceeded more than 4 folds. It was accordion folded from top down with no more than 4 folds. It was 50% neat and crisp.	Good Hand towel was accordion folded lengthwise. Did no exceeded more than 4 folds. It was accordion folded from top down with no more than 4 folds. It was 100% neat and crisp.
Dog Ear The top of hand towel is dog eared at the top.	Poor It was not dog eared.	Fair The towel was dog eared at least to 50% of functionality.	Good The towel was dog eared at least to 100% of functionality.
Envelope fold Folds were crisply made at all 4 corners. the 4th and final fold was stuffed into the envelope pouch with a dog ear sticking out for 2 finger unfolding. Final pack is symmetrical and neat.	Poor Folds are not crisp. 4th and final fold does not fit into the envelope pouch. Dog ear is missing or too small. The final pack is not no symmetrical and neat.	Fair Folds are 50% crisp. 4th and final fold fits into the envelope pouch with 75% of usefulness. Dog ear is present and adequate for 2 finger funfolding The final pack is 75% symmetrical and neat.	Good Folds are 100% crisp. 4th and final fold fits into the envelope pouch with 100% of usefulness. Dog ear is present and adequate for 2 finger funfolding The final pack is 100% symmetrical and neat.
