I sometimes disrupt the class in a variety of ways.	Level 3 I usually listen to the teacher or classmates. I very rarely disrupt the class.	Level 4 I always listen to the teacher or classmates. I never disrupt the class.
Preparedness	Level 1 I rarely bring my required materials -- pencil/eraser/notebook. I have had to borrow supplies three or more times this term.
