	Fair 2 pts	Good 3 pts	Excellent 4 pts
Cardiovascular	Fair Did not complete two aspects of cardiovascular fitness.	Good Completed one aspect of cardiovascular fitness.	Excellent Completed both aspects of fitness.
Task Items	Fair Completed under 8 items.	Good Completed between 8-9 items.	Excellent Completed all 10 items.
Board	Fair Board was all over the place and BINGO was not created by the activities completed.	Good Board was mostly organized and checked off pertaining to activities. BINGO was created one time.	Excellent Board was easy to follow and clearly marked off for teacher to follow including creating BINGO two times.
Class Time	Fair Student was a distraction during class and had to be reminded to stay focused and on task several times.	Good Student remained on task most of class and did what was asked of him/her.	Excellent Student worked efficiently during class and was a positive contributor during class time. Student went above and beyond what was asked and was even willing to help others.
Dress	Fair Student was not following PE/school rules in terms of dress, jewelry, etc.	Good Student was changed but had to be reminded to take jewelry off or put phone away.	Excellent Student was changed in appropriate PE attire with no jewelry on without being reminded.
