Student is OFF task. Student must be asked repeatedly about his/her assignments, The student may refuse help or play games instead of homework. The student is missing his/her supplies or computer is not charged.
Below Average

Student is OFF task more often than on task and requires significant reorientation to task. The student may only know assignments in 2 of 5 cases. The student may deny he/she needs help. The student almost always shows up to class without the necessary supplies (computer charged/ pencil/paper)
Below to Average

The student is on task more often than not, the student is basically organized but may need some support in this area. The student knows assignments in at least 3 of 5 cases. Student may need prompting to ask for help with work. The student often asks to go to locker for suuplies
Above average

The student is mostly on task and asks for help; the student demonstrates a good level of organization and knows what assignments are in 4 out of 5 cases. may not have all materials.
Excellent

The student stays on task for essentially the entire period; the student asks for help when needed; the student works toward completion/completes assignment. The student knows what the work is and stays organized and comes to study skills with all materials. Student works on additional educational projects when done with work.
Disruption of others

Poor

The student does not enter quietly and must be prompted to be quiet/seated/begin work. Student has more than 5 side conversations.
Below Average

The student does not enter quietly and continues side conversations (4-5 times).
Below to Average

The student is relatively quiet (enters talking), seated, having (3) side conversations and respects that self and others have work to do to grow in education.
Above average

The student is quiet, seated, having almost no (2) side conversations and respects that self and others have work to do to grow in education.
Excellent

The student is quiet, seated, having no side conversations and respects that self and others have work to do to grow in education.
