Student leaves the patient multiple times to gather various supplies.

Student gathers supplies prior to procedure but forgets 3-5 items and retrieves them in one trip.

Student gathers most supplies with the exception of 1-2 items and retrieves items in one trip.

Student gathers appropriate supplies including: clean gloves, sterile normal saline, chux, gauze dressing, tape, waste receptacle, clean sheet, gown, gloves, and mask with eye shield.

Student carelessly removes dressing, performs only a partial assessment, and does not remove gloves before beginning next step.
