	Novice The Novice or beginner has no experience in the situations in which they are expected to perform. The Novice lacks confidence to demonstrate safe practice and requires continual verbal and physical cues. 1 pts	Advanced Beginner Advanced Beginners demonstrate marginally acceptable performance because the nurse has had prior experience in actual situations. He/she is efficient and skillful in parts of the practice area, requiring occasional supportive cues. 2 pts	Competent The nurse is able to demonstrate efficiency, is coordinated and has confidence in his/her actions. For the Competent nurse, a plan establishes a perspective, and the plan is based on considerable conscious, abstract, analytic contemplation of the problem. Care is completed within a suitable time frame without supporting cues. 3 pts	Expert The Expert nurse has an intuitive grasp of each situation and zeroes in on the accurate region of the problem without wasteful consideration of a large range of unfruitful, alternative diagnoses and solutions. His/her performance becomes fluid and flexible and highly proficient. 4 pts
Patient Condition 15 % Recognize patient's condition is deteriorating	Novice Does not recognize patient deterioration until monitor alarms and patient has no vital signs.	Advanced Beginner Recognizes patient has or is deteriorating detoriating. Does not begin interventions until patient has no vital signs	Competent Recognizes patient condition is deteriorating by analizing vital signs. Begins to resuscitate patient when patient has no vital signs.	Expert Recognizes patient condition is deteriorating by analizing vital signs. And anylizes and implements interventions. Activates code team prior to loss of vital signs
Activation 15 % Activates Code Blue	Novice Needs to be directed to call 1100 to call code blue	Advanced Beginner Yells Code Blue, I need help. Fails to use phone to notify switch board of code blue and room number.	Competent Activates Code Blue, calls 1100, states Code Blue, gives room number	Expert Activates Code Blue, calls 1100, states Code Blue, gives room number. Begins to direct other team members in Code. Sends someone to get crash cart.
Begins CPR 15 % Begins CPR starting with compressions	Novice Begins chest compressions, no back board under patient, interrupts compressions, or compression are less than 100 per min.	Advanced Beginner Begins chest compressions, requires guidance and direction to place back board under patient.	Competent Does compressions, but Compression not fast enough or deep enough, must be reminded to push harder and faster. Back board is under patient.	Expert Places bed or stretcher in correct position to begin chest compressions. Places back board under patient. Places hands breast bone, performs compressions at a rate of 100 per minute, pushes 1 1/2 to 2 inches deep. Allows chest to fully recoil. Minimizes interruptions to compressions. Does all actions quickly and independantly.
Initiates Ventilations 15 %	Novice Needs assistance to locate ambu bag in crash cart.	Advanced Beginner Needs assistance in connecting ambu bag to O2 and how to place the bag. Not all ventilations make the chest rise	Competent Locates Ambu bag in crash cart. Removes ambu bag and connects to O2 source, turns on O2. Properly places bag on patient face with narrow end of mask over the nose. Uses the C placement to open airway and deliver 2 breaths in 10 seconds when compressions are stopped. Visible chest rise when ventilations are performed. May require minor assistance with any steps.	Expert Removes headboard from bead. Locates Ambu bag in crash cart. Removes ambu bag and connects to O2 source, turns on O2. Properly places bag on patient face with narrow end of mask over the nose. Uses the C placement to open airway and deliver 2 breaths in 10 seconds when compressions are stopped. Visible chest rise when ventilations are performed. Directs other team members to help with patient care. Does all actions quickly and independantly.
Communication & Teamwork 40 %	Novice Steps back and becomes an observer	Advanced Beginner Requires direction to perform tasks. Does not offer opinions or help independantly	Competent Functions as a member of the team. Follow leaders direction and offers suggestions	Expert Assumes leadership role and communicates directions to other members of the team. Uses closed loop communication
