Job Shadow Evaluation Introduction to Trades students will be evaluated on shop safety application, work ethic, professionalism and weekly reflection journals.
	Poor 10 pts	Needs Improvement 20 pts	Proficient 30 pts	Advanced 40 pts
Appearance 20 % Student arrives to job shadowing each week with appropriate Personal Protective Equipment (PPE) and trade ready attire	Poor Neglecting appropriate PPE; unsafe trade attire; disheveled in professional appearance (males).	Needs Improvement Clean, but careless trade appropriate attire; often neglects professional appearance	Proficient Properly dressed for job shadowing including PPE well groomed most of the time.	Advanced Always wears PPE and trade appropriate attire. Is professional In appearance.
Behavior 20 % Student arrives to job shadowing on time and is ready to go. Shows an interest and a desire to be there.	Poor Shows little enthusiasm, little interest in job shadowing exercise. Always tardy.	Needs Improvement Seems indifferent; lacks drive, often needs prodding. Seldom punctual.	Proficient Shows some interest in learning about the profession. Asks some questions. Punctual most of the time.	Advanced Shows much interest in learning about health care careers. Ask questions. Develops rapport with mentors. Always punctual. Anxious to get started.
Professionalism 20 % Student comes to his/her job shadowing assignment by being prepared and organized	Poor Student does not prepare for job shadowing by researching area of shadowing. Does not introduce self to mentor upon entering unit/department. Does not develop rapport with mentor.	Needs Improvement Student does some preparation for job shadowing by researching area. Does not introduce self to mentor upon entering unit.	Proficient Student does some preparation for job shadowing by researching area. Introduces self to mentor and shows interest in being on unit. Reflection Journal 40 % The student researches the weekly essential questions provided by instructor and completes a reflection of their reflection each week in their journal.
