Patient identification	Fail Patient was not appropriately identified at any time during the demonstration	Remediate Patient was identified by one identifier	Pass Patient was identified using 2 identifiers at the appropriate times throughout demonstration
Patient privacy	Fail The student failed to provide for the patient's privacy	Remediate The student provided for the patients privacy most of the time Violation:	Pass The student maintained patient privacy throughout the demonstration
Technical skill	Fail The student was not able to appropriately demonstrate skill or had greater than 5 self corrections or unidentified errors	Remediate The student was able to demonstrate skill with 3-5 self corrections or unidentified errors Corrections:	Pass The student successfully demonstrated skill with 1-2 self corrections or unidentified errors or no correction required
Patient interaction	Fail The student did not appropriately interact: failed to provide education related to the skill performed and did not converse with the patient	Remediate The student interacted with the patient at a minimum: only provided information about the skill performed or had minimal verbal interaction with the patient Discuss:	Pass The student fully interacted with the patient explaining all skills performed and conversing with the patient to make them at ease.
