Job Shadow Evaluation Health Science students will be evaluated on their appearance, behavior, professionalism and weekly reflection journals.

Appearance Student arrives to job shadowing each week with appropriate scrubs,shoes, name badge, hair, make-up.	Poor Untidy, carelessly dressed. Hair not pulled back (females). Unshaven, disheveled (males).	Needs Improvement Clean, but careless of appearance and grooming; often neglects appearance	Proficient Properly dressed for job shadowing: well groomed most of the time.	Advanced Always well groomed. Scrubs relatively free of stains, wrinkles.
Behavior Student arrives to job shadowing on time and is ready to go. Shows an interest and a want for being there.	Poor Shows little enthusiasm, little interest in wanting to be at job shadowing. Always tardy.	Needs Improvement Seems indifferent; lacks drive, often needs prodding. Seldom on time.	Proficient Shows some interest in learning about the profession. Ask questions. On time most of the time.	Advanced Shows much interest in learning about health care careers. Ask questions. Develops rapport with mentors. Always on time. Anxious to get started.
Professionalism Student comes to his/her job shadowing assignment by being prepared	Poor Student does not prepare for job shadowing by researching area of shadowing. Does not introduce self to mentor upon entering unit/department. Does not develop rapport with mentor.	Needs Improvement Student does some preparation for job shadowing by researching area. Does not introduce self to mentor upon entering unit.	Proficient Student does some preparation for job shadowing by researching area. Introduces self to mentor and shows interest in being on unit.	Advanced Student is prepared and has done research regarding area of job shadowing. Introduces self to mentor and shows much interest on unit. Asks questions when appropriate.
Reflection Journal The student researches the weekly essential questions provided by instructor and completes a reflection of their reflection each week in their journal.	Poor The journal entries are not clearly labeled with the appropriate dates or missing altogether. The weekly reflection is missing is not complete.	Needs Improvement The journal entries are clearly marked but are the essential questions are short and not complete. The weekly reflection is somewhat specific.	Proficient The journal entries are clearly labeled with the appropriate dates and the essential questions are answered . The weekly reflection is not specific.	Advanced The journal entries are clearly labeled with appropriate dates and the essential questions are answered thoroughly. The weekly reflection is specific and thorough.
