Punctuality	Poor I was slow getting changed.	Fair I was often slow to get changed before gym.	Good I was usually on time and ready to go when the teacher started.	Excellent I was on time for every gym class.
Preparedness	Poor I did not wear appropriate gym clothes, or gym shoes.	Fair I sometimes had my gym clothes.	Good I only forgot gym clothes/shoes once or twice.	Excellent I had appropriate gym clothes and shoes for each class.
Effort	Poor I demonstrate little or no effort to participate effectively.	Fair My effort is inconsistent and not always positively directed. I need constant encouragement.	Good My effort is good but it can be negatively influenced by the behavior of others.	Excellent I was actively involved, displayed a positive attitude, and worked to the best of my abililty.
