Exceptional Performance

Requests guidance and profits from suggestions, exhibits positive attitude about improving. Shows appreciation to teacher for helping.
Above Average

Willingly listens to constructive feedback, usually happy to make improvements without argument.
Average

Frequently takes correction / feedback well, sometimes needs help to keep a positive attitude. Appears a little annoyed.
Needs Improvement

Occasionally reluctant to take correction, and frequently acts resentful or upset. Will argue with teacher and make excuses.
Could Be Fired

Consistently resists correction, refuses to listen, may attempt to deny or blame, often becomes sullen. Would rather argue than fix problem.
Handling Problems/

Being Proactive on Assignment Completion

Exceptional Performance

Consistently deals with problems early and directly, independently works on solutions. Shows that she is proactive in anticipating needs. All assignments are caught up with no prompting needed. Will ask for make-up work after missing class.
Above Average

Usually deals with problems and works on solutions, sometimes asks for guidance. Mostly stays current with staying on top of getting assignments done. Mostly gets make-up work after missing class.
Average

Often deals with problems and will try to solve problems if encouraged. Needs adult reminders often of work assignments that are missing. Often needs to be reminded of getting missing work after missing class.
Needs Improvement

Occasionally shuts down instead of facing problems. Sometimes will take direction and work on a solution. Mostly dependent on teacher/parent to keep her on track with assignments or make-up work. Little effort to get make-up work after missing.
Could Be Fired

Rarely faces up to problems, refuses to work on solutions, denies the problem or blames others. Is not able to keep up with doing class work or turn in assignments independently. Never asks for make-up work.
Manages time on task

Exceptional Performance

Is consistently punctual to classes and begins class by writing assignment in planner and starts bell ringer activity with no prompts from teacher. Does not get book out at inappropriate times.
Above Average

Is mostly punctual to class and writes in planner after the teacher reminds the class to get started. Sometimes needs to be reminded to put book away. Has positive attitude about request.
Average

Once in a while is a few minutes late. Rarely writes in planner unless prompted to do so. Is often asked to put reading book away. But has good attitude about it.
Needs Improvement

Inconsistent in getting to class on time. Needs verbal prompt to write in her assignment book and needs to be told to put book away most days. Shows that she is annoyed by being asked to do so.
Could Be Fired

Frequently not in room and ready to work at the start of class. Resists writing in planner and gets mad if told to put book away.
Works cooperatively with others

Exceptional Performance

Consistently listens and shares with others. Supports the efforts of group and makes a positive difference in keeping the group working productively.
Above Average

Usually listens and shares with others but sometimes seems not as focused. Mostly supportive of others but occasionally will try to push the group to do things her way.
Average

Often shares and listens to others but not as supportive to differing ideas. Sometimes is not a good team member by wanting to only do it her way but tries not to make waves.
Needs Improvement

Regularly needs to be reminded to share and listen to others in group. Needs prompted to stay positive. Often makes waves or has conflict.
Could Be Fired

Displays a negative attitude and does not want to share and listen to others. Creates conflicts in the group more than helping work productively.
Organization

Exceptional Performance

Has materials organized in a system with folders and assignments/papers where they belong.
Independently manages and consistently brings materials to class.
Above Average

Is mostly maintaining a work system but occasionally forgets to bring all materials to class.
Average

Inconsistently keeping materials organized and often forgets needed items for class.
Needs Improvement

Occasionally tries to use system but papers are not kept neatly in correct place within system. Regularly needs to retrieve lost items in locker.
Could Be Fired

Does not try to use any organization system and does not care. Rarely has correct materials in class.
