This project will the clinical documentation of the recognized responsive behavior(s), the reflection on possible triggers and ways to mitigate the triggers, pharmacological and non-pharmacological interventions, a succinct SBAR description, and recommendations for ongoing care to help reduce the client's responsive behaviors.
