Community Assessment - Profile 
To understand the impact of the community setting upon the well-being of community members, the group will identify: 1. A health Issue of importance to an identified community. 2. Develop an evidence -based intervention to address the intervention. Focus is the " larger community". Profile includes: 1. the identified database, problems. issues/challenges, plan, and evidence based intervention.
