Observation of Community 25%	Excellent The observation is thorough and all issues are addressed. Very descriptive, well-written and all directions followed.	Good Health issue/issues are stated or addressed but, lack detail. The community is primarily described.	Fair Health issues are addressed, a clear picture of the community is not accomplished.	Poor Health issues addressed, vague and non-descriptive.
Social Characteristics 25%	Excellent All census data is provided for each category with references.	Good At least 7 sections are described.	Fair At least 5 sections are completed.	Poor Less than 5 sections or inaccurate information.
Issue of Concern 25%	Excellent Problem is clearly identified with the etioloty and proof of concern among community members. Intervention is based on research and understanding of community.	Good Problem identified, but not in detail, intervention is too general.	Fair Problem is slightly identified, intervention not clearly articulated.	Poor Vague problem identifcation, no intervention plan provided.
References 25%	Excellent Use of at least 10 references, all within the last 2 years and correctly formatted APA style, and all evidence-based.	Good Between 7-9 references. Not all evidence -based.	Fair Between 5-7references, 50% or more not evidence-based, and not formatted APA.	Poor Less than 5 references, APA format not followed, and/or not evidence based.
