Students Behavior Check List 
This rubric allows students with behavioral problems to self monitor their own behavior and see how they are acting and performing on a day to day basis. The student checks how they feel and then the teacher would collect it to see if it matched what they thought at the end of science class. The goal is to see improvement with the use of the checklist rubric.

Poor

I did not want to work and chose not to pay attention in class. I was not looking at the teacher or listening during class.

I was paying attention for parts of the class but not all of it. I was playing or drawing part of the time.
(The teacher had to prompt me more than twice.)
Good

I was listening well in class. I was looking at the teacher and knew what to do during class.
(No more than 2 prompts from my teacher.)

How well did I participate in class?

Poor

I didn't want to do my work or work with my peers on group assignments during class.
I did not get help from my teacher.
(I was talked to several times or had to leave class.)
Fair

I was participating but not giving it all my effort. I did not raise my hand or do all I was asked to do.
(The teacher had to prompt me more than twice.)
Good

I fully participated. I did all the work I was asked to do. I worked well with others. I raised my hand and joined in. I got help when I needed it. I had no refusals.
(No more than 2 prompts)
My Attitude

Was I respectful to others?

Poor

I had a negative attitude in class. I did not accept correction from my teacher. I argued. I chose not to listen or follow directions.
Fair

I was struggling and frustrated with class but tried my best to remain on task. I asked for help or calming time and tried to get my work done.
Good

I was respectful to others. I listened. I accepted correction. I followed directions.
Independence

Was I able to use fourth grade behaviors?

Poor

I had refusals or shut-down. I was not part of class today.
Fair

I wanted to refuse but calmed down and got through class. I did not always ask the classroom teacher for help and wanted Ms. T or Mrs. D or Mrs. Orr instead.
Good

I went to all classes. I tried everything on my own before I asked the classroom teacher for help. I did not look to Ms. T, Mrs. D, or Mrs. Orr for help.
